Event description
Employee killed when struck by metal pan
Investigation abstract
Employee #1 was called to a job site to repair bushings on a roller assembly to which a pan was attached. The pan was removed from the bowl by a crane and Emplo yee #1 made the repairs. When the pan was being lowered back inside the bowl, it became wedged inside and Employee #1 tried to free it. When it was freed, the p an came forward, striking him in the abdomen. Employee #1 died of internal injur ies.
