Employee killed in fall from scaffold
Investigation abstract
At approximately 3:30 p.m. on August 6, 1986, Employee #1, who worked for an out side contractor, was doing maintenance and repair work for an automobile dealers hip. He was using two 6 foot sections of a mobile scaffold to change the fluores cent lights in the ceiling of the dealership's service department. Employee #1 f ell approximately 8 feet from the scaffold, landing on an automobile that was on a Horst light lift, and then fell approximately 6 feet to the concrete floor. H e died of massive head injuries. The scaffold had no guardrails.
