Employee killed in fall from ladder
Investigation abstract
At about 1:20 p.m. on October 16, 1986, Employee #1 had completed building a con crete block wall. He used a stepladder to exit the scaffold at the 12 foot level . He had descended the ladder to approximately the 6 foot level when he slipped and fell. The ladder was located at the edge of a 4 foot deep loading dock and E mployee #1 fell into the dock, striking his head on the concrete floor. He was k illed.
