EMPLOYEE KILLED WHEN STRUCK BY FALLING CONCRETE PIPE

Investigation abstract

A PRE-STRESSED CONCRETE PIPE WAS BEING LIFTED, IN A VERTICAL POSITION, BY A FORK LIFT. EMPLOYEE #1, WHO WANTED THE PIPE TO BE LAID HORIZONTALLY ON THE FLOOR, INS TRUCTED THE LIFT OPERATOR TO LOWER THE PIPE. WHEN THE PIPE HAD BEEN LOWERED ENOU GH SO THAT ITS EDGE CONTACTED THE FLOOR, THE WEIGHT AND BALANCING OF THE PIPE WA S TRANSFERRED FROM THE FORKLIFT TO EMPLOYEE #1. HE LOST CONTROL OF THE PIPE AND IT FELL, CRUSHING HIS ABDOMEN. HE WAS KILLED.
