Employee dies on the job

Investigation abstract

Employee #1 climbed onto a scissor lift after getting parts at the tool crib. Th e employee then flopped into a sitting position on his tool bucket and rested hi s head into the corner of the guardrails, looking up. When Employee #1 sat down, he dropped the bolts from his left hand onto the floor. A coworker picked up th e bolts and threw them into Employee #1's lap. Employee #1 did not respond. Then Employee #1's partner raised the lift, thinking that Employee #1 was joking. Th e partner began working but did not see Employee #1 working. When the partner lo oked down, Employee #1's body was on the floor of the lift, sliding out the entr ance and falling approximately 28 feet. Employee #1 died.
