Employee dies in confined space
Investigation abstract
At approximately 2:30 a.m. on March 7, 1986, Employee #1 was assigned to prepare for a 4:00 a.m. loading of oil from vat #14 into a tank wagon. Employee #1 open ed the man hatch of the H2 blanketed vat to check on the amount of oil or to sho w a sample. At approximately 5:00 a.m. he was found dead at the bottom of the va t.
