SKULL FRACTURED BY FALLING FORK LIFT TRUCK

Investigation abstract

THE FORK END OF A FORK-LIFT TRUCK HAD BEEN LIFTED 12 TO 18 INCHES BY A HOIST. EM PLOYEE #1 INTENDED TO DO SOME TAC WELDING ON THE PINS UNDER THE FORKLIFT FORKS. WHILE EMPLOYEE #1'S HEAD WAS UNDER THE FORKS, THERE WAS REPORTABLY A LOUD NOISE, FOLLOWED BY THE FORKS FALLING TO THE GROUND. THE EMPLOYEE'S HEAD WAS CAUGHT UND ER THE FORKS. HE SUFFERED A FRACTURED SKULL AND WAS KILLED. THE ACCIDENT OCCURRE D AT ABOUT 9:45 AM ON JUNE 24, 1986.
