EMPLOYEE CRUSHED BY FALLING BOX OF METAL PARTS

Investigation abstract

EMPLOYEE #1 ATTEMPTED TO CLIMB A STACK OF CARDBOARD BOXES CONTAINING METAL PARTS . AS HE WAS CLIMBING HE INADVERTENTLY PULLED THE TOP BOX OVER ONTO HIMSELF. THE FALLING BOX STRUCK HIM IN THE HEAD, CRUSHING HIS HEAD AND KILLING HIM.
