EMPLOYEE #1 WAS STRUCK IN THE LEG AND HIP AND KILLED WHEN A STEEL CYLINDER, USED AS AN INSIDE CONCRETE FORM FOR MAKING CONCRETE PIPE, FELL FROM THE CRANE HOOK, STRUCK THE GROUND AND FELL OVER ONTO HIM. THE CYLINDER IS 7 1/2 FEET TELL AND WE IGHS APPROXIMATELY 1000 LBS.
