Employee killed in fall from ladder
Investigation abstract
Employee #1 was approximately 7 ft above a concrete floor, working from a 24 ft extension ladder that had been placed on a 14 ft 4 in. metal joist. He either lo st his balance or the ladder slipped and he fell backward, striking the bottom o f his left rib cage against a large paper roll. He then fell to the concrete flo or, striking the occipital bone in his skull (proximal and posterior to his righ t ear). Employee #1 was killed.
