Investigation abstract
EMPLOYEE #1 WAS DRIVING A CEMENT BREAKER FROM ONE JOB SITE TO ANOTHER OVER A STA TE HIGHWAY. THE RIGHT SET OF REAR WHEELS BROKE LOOSE FROM THE HUB, CAUSING THE T RUCK TO ROLL OVER. THE BOOM OR THE CONCRETE BREAKER CRUSHED EMPLOYEE #1'S HEAD B ETWEEN THE CONCRETE BREAKER AND THE PAVEMENT, KILLING HIM.
