Event description

Employee killed when struck by falling floor plate

Investigation abstract

On November 25, 1987, Employee #1, a driller who had assisted with rigging opera tions in the past, was helping to rig up drilling rig #4 at the ITS Simoneaux we ll #23, in Louisiana. He was standing on top of two motor sheds that had been pu t in place and was helping to place a large rig floor plate that measured 3 1/2 by 13 ft and was being hoisted from ground level by a Link Belt 118 crawler cran e. As it neared Employee #1 the load shifted and the floor plate fell, striking Employee #1 and crushing him to death.
