EMPLOYEE KILLED WHEN STRUCK BY VACUUM TRUCK
Investigation abstract
Employee #1 was on a three-wheel industrial-use bicycle. As he came near the cor ner of a warehouse building, a vacuum-type truck approached from the opposite si de. Employee #1 attempted to stop; however, in doing so, he fell beneath the rea r wheels of the truck. The rotating wheels of the truck crushed his head killing him.
