Employee killed in fall through roof opening
Investigation abstract
Employee #1 and a coworker were installing nailers around a roof skylight. Emplo yee #1 started walking toward the northeastern corner of the opposite side of th e roof, where materials were standing. The coworker was on his knees at the west ern end of the bay #4 skylight. Employee #1 turned to the coworker while he was walking and informed him that he was going after more lumber. As soon as he turn ed around he stepped into an uncovered gravity vent roof opening and fell 46 ft to a concrete floor. He sustained multiple internal and head injuries and died e n route to the hospital.
