Employee killed in fall through skylight
Investigation abstract
Employee #1 and coworkers were on a flat roof, assisting in the helicopter remov al of roof fans. As they backed away from a fan that was being lifted, Employee #1 tripped and fell sideways through a 50 in. by 90 in. plastic-covered skylight . He fell 30 ft to a concrete floor, sustaining fatal head injuries. Employee #1 died at the site.
