Employee killed when struck by falling headache ball
Investigation abstract
Employee #1, an experienced crane operator, was performing flagging operations w hen the hoist line was two-blocked and the 190 lb headache ball was drawn up to the boom nosepoint, creating extreme stress on the wire rope. This caused the ba ll to fall and strike Employee #1 in the head. He was killed.
