WET FLOOR CAUSED EMPLOYEE TO SLIP AND FALL, STRIKING HEAD

Investigation abstract

EMPLOYEE #1 WAS ENGAGED IN CLEANING OUT LINES OF CEMENT. THE ENGINE CONTROL FOR THE WATER PUMP STUCK OPEN, CAUSING THE PUMP TO ACCELERATE AND BRING ABOUT AN INC REASE IN WATER PRESSURE. WATER CASCADED ONTO THE ENTIRE FLOOR. EMPLOYEE #1, WHIL E ATTEMPTING TO GET AWAY FROM THE WATER, FELL OVER BACKWARDS AND STRUCK HIS HEAD BEHIND THE RIGHT EAR. HE WAS KILLED.
