Employee killed in fall from ladder
Investigation abstract
Two employees working for an electrical contractor had set a 12.2-meter-long alu minum extension ladder against a utility pole. They were setting up a pulley wit h an attached chain to pull service conductors through a 76-millimeter-diameter conduit. One of the employees climbed to the top of the ladder with the 6.8-kilo gram chain. As he reached up to run the chain through the U-bolt, he lost consci ousness, slumping against the ladder and then falling about 9.1 meters to the gr ound. He died of his injuries.
