STRUCK BY SWINGING LOAD; FELL 35 FEET TO FLOOR

Investigation abstract

EMPLOYEES #2 AND #3 WERE RIGGING A LOAD OF REBAR TO BE HOISTED TO A HIGHER LEVEL . THE LOAD SWUNG WHEN IT WAS HOISTED. EMPLOYEE #3 WAS STRUCK AND KNOCKED OVER TH E EDGE OF A RAMP CUTOUT AS HE WAS TRYING TO GET OUT OF THE WAY OF THE LOAD. HE F ELL ABOUT 35 FEET TO A CONCRETE FLOOR. EMPLOYEES #1 AND #2 WERE ONLY BRUISED. TH E LOAD WAS RIGGED WITHOUT THE HOOK BEING CENTERED OVER THE LOAD TO PREVENT SWING ING.
