EMPLOYEE #1 FELL INTO A OPEN ELEVATOR SHAFT ON A CONSTRUCTION SITE. THE SHAFT WA S NEITHER COVERED NOR SURROUNDED BY A GUARDRAIL. HE FELL APPROXIMATELY 35 FEET I NTO THE ELEVATOR PIT. HE WAS KILLED.
