Employee's leg amputated after becoming caught in auger
Investigation abstract
Employee #1, age 19, was using a high pressure washer to clean the inside of a M epaco mixer-blender used for meat processing. To facilitate cleaning, Employee # 1 climbed to the slide at the back of the machine and was washing with one hand while maintaining his balance by holding onto the mixer guardrail with his other hand. The slide is sloped toward the inside of the mixer, and while cleaning Em ployee #1 lost his balance and fell feet first into the rotating auger/agitators . Employee #1's left leg from his foot to his thigh became caught between one au ger and the side of the mixer. He was hospitalized and his left foot was later a mputated above the ankle. Employee #1 also suffered a fractured right leg.
