Event description
Employee killed when struck by pipe
Investigation abstract
Employee #1 was working near where his employer had installed a Chic-San swivel hammer connection pipe from the tubing wing assembly valve at the top of the Chr istmas tree to a Swaco choke assembly. A nipple connected the choke assembly to the hammer connection pipe. When the nipple in the pipeline broke, the pipeline moved violently in the air, bending the tubing wing assembly and striking Employ ee #1. He was killed. The pressure inside the Chic-San hammer connection pipe an d the pipeline was 4,650 psi.
