Event description

Employee killed when struck by poles

Investigation abstract

At approximately 7:30 p.m. on January 7, 1989, Employee #1 was riding in the bac k of a pole truck. He was standing behind the driver and holding onto the headac he rack. The truck's hook was not snubbed to the rear of the truck and the winch was engaged without anyone realizing it. The winch pulled the hook up to the to p pulley and then pulled the A-frame poles over the top. The poles struck Employ ee #1, killing him.
