Event description
Employee killed when fell from ladder
Investigation abstract
An employee was killed while descending a bracket scaffold system that was insid e a storage unit. The employee was moving from the sixth rung to the fifth rung of the scaffold using an unsecured 8-ft wooden ladder. The scaffold rungs were 1 0 ft between levels and level 1 was approximately 5 ft 5 in. from the floor. Whi le trying to reach the top of the 8-ft ladder, the employee slipped and fell 5 f t 5 in. to the floor of the storage tank.
