EMPLOYEE #1 WAS OPENING DOUBLE DOORS ON A CONTAINER. THE EMPLOYEE OPENED THE RIG HT DOOR OF THE CONTAINER. HE HAD OPENED THE LEFT DOOR ABOUT HALFWAY WHEN A BALE OF BURLAP, WEIGHTING ABOUT 900 POUNDS FELL ON HIM. HE WAS CRUSHED TO DEATH.
