WHILE THE WELL TUBING WAS BEING PRESSURIZED FOR LEAKS, THE COUPLING ON THE TUBIN G RUPTURED. THE RESULTING FRAGMENTS STRUCK EMPLOYEE #1 IN THE CHEST, FATALLY INJ URING HIM.
