Event description

Employee killed when buried by fertilizer in storage bin

Investigation abstract

Employee #1 entered a storage bin containing bulk granular ammonium sulfate fert ilizer to break up clods. A large amount of fertilizer broke loose, completely c overing Employee #1 and forcing his foot down through the opening at the bottom of the bin. The rescue operation took about 20 minutes. Employee #1's mouth and nostrils were filled with fertilizer and he was pronounced dead at the scene.
