Employee dies after fall from ladder
Investigation abstract
At 1:30 p.m. on July 15, 1987, Employee #1 was pulling electrical wire through a conduit. He was about 6 ft above the floor, standing on an 8 ft ladder, when th e ladder came free of the fish tape holding it in place. Employee #1 lost his ba lance and fell to the concrete floor. He struck his head on the floor and lost c onsciousness for 2 or 3 seconds. He was conscious when EMS personnel arrived, bu t died at 12:00 midnight on July 19, 1987.
