CONSTRUCTION EMPLOYEES THROWN AND CRUSHED BY MACHINE BUCKETS

Investigation abstract

EMPLOYEES #1-3 WERE REPAIRING AND REPLACING WORN AND BROKEN DRIVE CHAINSAND BUCK ETS IN AN EXTRACTOR MACHINE. AS THE EMPLOYEES WERE JOGGING THE DRIVE CHAIN, THE SHEAR PINS BROKE, CAUSING THE BUCKETS TO DROP 10 FEET. A LOOSE BUCKET FELL ON EM PLOYEE #1, CRUSHING HIM. EMPLOYEE #2 WAS THROWN OUT OF A BUCKET AND CAUGHT HIS L EG BETWEEN A BUCKET AND STEEL BEAM. EMPLOYEE #3 WAS THROWN DOWN THE UNLOAD HOPPE R CAUSING BUMPS AND BRUISES. EMPLOYEE #1 WAS FATALLY INJURED. EMPLOYEES #2 AND # 3 WERE HOSPITALIZED.
