Employee killed when struck by crank arm on cutter machine

Investigation abstract

Employee #1 was on a harp cutter machine with his head inside of it, looking dow n. The crank arm that raises and lowers the cutter struck the back of his head, pushing it down into a steel cross-member. Employee #1 was killed.
