Investigation abstract

A MAINTENANCE EMPLOYEE WAS CLEANING THE PIT OF AN ELEVATOR SHAFT. THE ELEVATOR C ONTROLS WERE NOT LOCKED OUT. THE ELEVATOR CAR LOWERED AND CRUSHED THE EMPLOYEE.
