Employee killed in fall from ladder or storage tank
Investigation abstract
Employee #1 and three coworkers were measuring two 28 ft tall storage tanks in p reparation for installing fixed stairs and walkways. Employee #1 fell either fro m some type of elevated ladder or from the top of the storage tank. He was found by a coworker lying in a pool of blood on the concrete deck between the two tan ks. Employee #1 was killed.
