Employee killed in fall from flat roof

Investigation abstract

Employee #1 was working on a flat roof installing metal strips for face trim. He either tripped over ridges on the roof or slipped, falling head first to the as phalt driveway. Employee #1 was killed. He was not provided any fall protection.
