Event description
Employee crushed and killed between truck dump box and frame
Investigation abstract
Employee #1 was working by himself repairing a truck frame on a 1974 Ford 24 ton from the prime contractor who was passing by. dump truck, PSC-5527, with a 14 foot box of 10 yard capacity, and 1970 Heil hoi st. A trip wire ran the length of the frame to the end of the box to prevent the box from being raised too high. A manually-operated support arm was available t o prevent accidental lowering of the box during repair work. Employee #1 was pos itioned above the last axle with his head on the passenger's side and his legs r esting on the wheels of the driver's side. Apparently, he accidentally pulled th e trip wire, causing the box to lower and pin him to the frame. Employee #1 died of a crushed head and lower torso. He was discovered around 3 p.m. by a worker
