Employee killed in fall from ladder
Investigation abstract
On May 4, 1988, Employee #1 was ascending an approximately 20 ft long fixed meta l ladder. About halfway up he dropped some papers into an open area behind the l adder, where they landed on a false ceiling. Employee #1 tried to retrieve the s cattered papers by holding onto the fixed ladder and leaning over toward them. H e lost his balance or grip and went through the false ceiling. He fell over 10 f t onto a concrete stairway landing platform between some risers. Employee #1 die d on July 14, 1988.
