Investigation abstract

EMPLOYEE #1 WAS MOVING CARDBOARD CARTONS FILLED WITH WOODEN ROOM DIVIDERS. FOUR FELL OVER, KNOCKING HIM TO THE FLOOR. HE SUSTAINED A SPINAL INJURY WHICH RESULTE D IN HIS DEATH.
