Event description
One killed, three injured when struck by drill collar
Investigation abstract
At approximately 2:30 a.m. on September 6, 1986, Employees #1 through #4 were us ing a rig elevator to move drill collars from the rotary table on an oil drillin g rig. When a shear pin broke, the elevator latch came open and a drill collar f ell onto the workers. Employee #1 suffered severe head and back injuries and lat er died. Employee #2 dislocated a shoulder, Employee #3 bruised his neck, and Em ployee #4 cut his finger.
