Event description

Employee killed when crushed by falling truck bed

Investigation abstract

Employee #1, a flatbed truck driver, was dumping debris at a landfill. The truck bed was partially raised when it stopped for some reason. Employee #1 apparentl y noticed a severed steel control cable leading to the power take-off. He had po sitioned himself between the raised bed and the truck frame to repair the cable when the bed lowered, crushing and killing him. Employee #1 may have moved the h ydraulic linkage lever while under the raised bed. There were no witnesses to th e accident.
