Employee injured in fall from job-made scaffold board
Investigation abstract
Three employees were working 22 feet above ground level from a pump jack scaffol d. The scaffold board, which was job-made, broke in two near the mid-point. Empl oyee #1 fell and fractured his hip. The scaffold board, consisting of 2 by 4s an d wafer boards, was 12 feet long by 2 feet wide. Two by fours and wafer boards a re not the equivalent of scaffold grade planking. The scaffold board was also ov erloaded for its given span.
