Employee killed when struck by load of PVC pipe
Investigation abstract
Employee #1 was securing a load of 18 in. in diameter and 20 ft long PVC pipe wh en the load became dislodged and struck him on the head. Employee #1 was killed. He had not been trained by the employer in loading and securing the load.
