Employees killed in head-on collision at construction site
Investigation abstract
Employees #1 and #2 were in separate vehicles transporting asphalt to a paving o peration on a road construction site. As the employees attempted to pass each ot her, exiting or entering the paving site, they collided head-on. Both employees died. The day was clear and warm. There was good visibility on a relatively stra ight road. The employer failed to establish a pre-planned traffic pattern on the job site to eliminate congestion and hazardous conditions and provide operators with clear instructions and signage regarding job traffic patterns.
