EMPLOYEE CRUSHED TO DEATH BY SCREW CONVEYOR ABOARD A SHIP
Investigation abstract
THIS ACCIDENT TOOK PLACE ABOARD A GRAIN-CARRYING SHIP EQUIPPED WITH WING TANKS O N EACH SIDE OF THE SHIP. A SCREW CONVEYOR RUNS THROUGH A LONGITUDINAL TROUGH IN EACH WING TANK. TWO WING TANKS WERE BEING WASHED. SIMULTANEOUSLY, A MARINE CHEMI ST AND A SHIP-REPAIR COMPANY EMPLOYEE WERE INSIDE ONE OF THE OTHER WING TANKS NO T BEING WASHED. EMPLOYEE #1 WAS STANDING THE CONVEYOR TROUGH WHEN ONE OF THE SHI P'S CREWMEN TURNED THE CONVEYOR ON. HE WAS CAUGHT AND CRUSHED TO DEATH BY THE AU GER. THE SHIP'S CREW APPARENTLY WAS UNAWARE OF THE THE TWO MEN'S PRESENCE IN THE WING TANK. A LOCKOUT PROCEDURE WAS IN EFFECT FOR THE WORKERS WASHING OUT THE OT HER TANKS.
