STRUCK BY FLYING CRANK HANDLE

Investigation abstract

EMPLOYEE #1 STARTED A MOTOR, WHICH POWERED A ROTATING SHAFT, WITHOUT REMOVING A CRANK FROM THE END OF THE SHAFT. THE CRANK WAS THROWN FROM THE SHAFT AND STRUCK EMPLOYEE #1 IN THE HEAD, INFLICTING MULTIPLE FRACTURES AND CAUSING HIS DEATH.
