Event description

ELECTRIC SHOCK - CONTACT WITH ACCIDENTALLY REENERGIZED LINE

Investigation abstract

AN EMPLOYEE (EMPLOYEE #1) WAS COILING UP WIRE ON A FARM. THE LINE WAS DEENERGIZE D AT THE TIME. THE GENERAL FOREMAN AT A REMOTE SITE TOLD A SECOND EMPLOYEE TO PU T THE JUMPER FROM THE NEW LINE TO THE TAP. WHEN THIS WAS DONE, IT REENERGIZED TH E OLD LINE BECAUSE A JUMPER WAS STILL ON THE OLD LINE. THE FOREMAN DID NOT SEE T HAT THE JUMPER WAS STILL CONNECTED TO THE OLD LINE; BUT WHEN HE SAW THE FLASH, H E REALIZED WHAT HAD HAPPENED. HE JUMPED INTO HIS PICKUP, DROVE TO WHERE HE LAST SAW EMPLOYEE #1, AND SAW A THIRD EMPLOYEE GIVING CPR TO EMPLOYEE #1. EMPLOYEE #1 WAS ELECTROCUTED.
