EMPLOYEE #1 WAS STANDING ON THE RIGHT SIDE OF A MOVING FIRE PUMPER TRUCK. HE LOS T HIS BALANCE AND FELL BACKWARDS OFF THE TRUCK PLATFORM. HE LANDED, HEAD FIRST, ON THE PAVEMENT AND WAS KILLED.
