Employee's finger amputated by filter auger
Investigation abstract
Employee #1 was removing an auger from a line filling funnel. He failed to lock the equipment in place, even though lockout was possible 1 ft away from the funn el. The equipment was run by a computer which sensed that the line run had not f inished. It started up and the auger amputated Employee #1's left index finger.
