Investigation abstract

EMPLOYEE #1 WAS CLEANING CHUNKS OF CORN OFF THE GRATE LOCATED BEHIND ELEVATOR "C ", BY THE WEST CONCRETE STORAGE BIN. THE THUMB ON HIS LEFT HAND ENTERED THE OPEN ING ON THE CORN AUGER AND WAS COMPLETELY AMPUTATED.
