FINGER AMPUTATED IN METAL BAND SAW

Investigation abstract

EMPLOYEE #1 WAS OPERATING AN AUTOMATIC HYDRAULIC BAND SAW, USING IT IN THE MANUA L MODE. HE REVERSED THE BLADE DIRECTION, BUT FORGOT TO SHUT THE SAW OFF. HE REAC HED INTO THE DANGER ZONE TO MEASURE THE DEPTH OF THE CUT AND GOT HIS GLOVE CAUGH T IN THE RUNNING BLADE. HIS RIGHT INDEX FINGER WAS AMPUTATED AT THE FIRST JOINT.
