EMPLOYEE FELL WITH LADDER; FRACTURED ARM AND TOE

Investigation abstract

AT APPROXIMATELY 9:30 AM ON JANUARY 10, 1986, EMPLOYEE #1 WAS USING AN EXTENSION LADDER TO CLIMB TO THE TOP OF A MANUALLY CONTROLLED OVERHEAD PULL DOOR TO REPAI R A SPRING. THE SPRING WAS LOCATED ABOUT 12 FEET ABOVE FLOOR LEVEL. THE LADDER W AS SET AT A VERY SHARP (UNSTABLE) ANGLE. WITHOUT ANY WARNING THE LADDER FEET BEG AN TO SLIDE OUTWARD, CAUSING THE LADDER TO FALL. EMPLOYEE #1 WRAPPED HIS RIGHT A RM AROUND THE LADDER WHILE HE RODE IT TO THE FLOOR. HE LANDED ON HIS RIGHT SIDE, FRACTURING HIS RIGHT ARM AND THE BIG TOE ON HIS RIGHT FOOT.
