Investigation abstract

ON JULY 26, 1985, AT APPROXIMATELY 5:30 PM EMPLOYEE #1 WAS CUTTING A PIECE OF BO ARD FOR A CUSTOMER ON THE RADIAL ARM SAW LOCATED IN THE YARD AREA OF THE BUSINES S. WHILE CUTTING, THE WOOD SLIPPED AND PULLED EMPLOYEE #1'S HAND INTO THE SAW, R ESULTING IN THE AMPUTATION OF HIS THUMB.
